You know that saying had to come about because it actually happened somewhere. I don't think you come up with "the shit hit the fan," out of your imagination. Someone actually experienced shit hitting a fan one time. It's not like you hear about "the sandwich bread bag of urine hit the sidewalk from the fourth floor window" just randomly.
